My students have their fist name in katakana in the middle and surname is at the bottom.
Reason: It is easier for Japanese people to read it and usually they want to go by first name. That is why I think that is a good format.
That is a part of culture. In Japan, we never put our first name on tare. Some have their first name as well if they have common last name.

Some of the members of my dojo had a proposal to design a logo for our zekken, since many other dojos, including dojos in our regional federation, have unique logos. We submitted it to our sensei, who said, "Dojos in Japan do not have logos, we will not have logos on our zekken." End of discussion. Our regional federation zekkens have the federation logo on them, however. Confusing?
In some respects, I can understand that, in others, though, some means of identifying a dojo through its logo is helpful. When you see two kenshi who are from dojos with similar names, which one is from the dojo you are looking for?
One other thing, while there's no set layout, my original zekken has my dojo name across the top, my surname in English on the bottom, and my surname in katakana/hirakana in the middle. It's only an approximation of my name, and given how I feel about it now, I would have left that area blank, unless we were allowed to add a logo
